Graduate Certificate Completion Review Forms
Students who enrolled in the Graduate Certificate in Educational Technology or Graduate Certificate in Online Teaching and Learning as a stand-alone credential or as a complement to their MAET degree program must submit a Completion Review form to have their Graduate Certificate officially conferred on their transcript.
Once you have successfully completed all 3 required courses for your Graduate Certificate Credential, please complete the appropriate Completion Review form and email it to email@example.com or, if you are on campus, drop it off for us. Upon verification that all requirements are met, the program code will be marked as conferred on your student record. You will need this form on file in order to have the certificate listed on your transcript.
Please note two important updates. First, conferral of these graduate certificate credentials does require students to have achieved a cumulative 3.0 GPA in the required courses. Second, students must get their program advisor’s signature before submitting the completion review form for review by the MAET program. If you are in the MAET degree program, Angelica Kim must sign your form. If you are in another degree program, your program advisor must sign the form. Angelica will also sign all completion reviews for students enrolled in the Graduate Certificate in Ed Tech and Online Teaching and Learning as stand-alone credentials.
Graduate Certificate Intent to Enroll Forms
Complete this form to declare your intent to complete the graduate certificate and you are a student currently enrolled or newly admitted to another master’s degree program in the MSU College of Education.
After completing the certificate courses, you can request an updated transcript. It costs $5, and you can pay with credit card, check or money order. With credit card, you can request the transcript online. You will need your MSU Net ID and password (that is, your email login information) or your student ID, social security number, and date of birth.
